THE RESTORATIVE RMT COMMITMENT

We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the goods and services provided through The Restorative RMT. All policies are in accordance with the Massage Therapy Act, 1991, CMTO Standards of Practice, Personal Information Protection and Electronic Documents Act, 2000, and the Personal Health Information Protection Act, 2004.

  • Get payment for your treatment (from OHIP, WSIB, private insurer or other if required)

ACCESS TO YOUR INFORMATION

  • Upon request, you have the right to confirm whether or not therestorativermt.com maintains or controls any Personal Information relating to you and whether such Personal Information has been used or disclosed by therestorativermt.com.
  • Further, if therestorativermt.com. does maintain or control any Personal Information relating to you, you have the right, upon request, to access such Personal Information and to request correction of any erroneous or out-of-date Personal Information about you. In some limited circumstances as prescribed by law, we may not be able to grant you access to your information.

THIRD PARTY ACCESS

The College of Massage Therapists of Ontario may inspect records as a part of their regulatory activities in the public interest as mandated by law. Various government agencies (eg. Canada Customs and Revenue Agency, Information and Privacy Commissioner, Human Rights Commission, etc.) also have the authority to review documentation as a regulatory body. Your written consent will be required before submitting personal health information to non-regulatory bodies (eg. Insurance, Legal Council, etc.).
